THE FORM BELOW IS AMENDED FROM THE ONE USED IN GREATER LONDON AND CAN PROVIDE YOUR LOCAL POLICE WITH MUCH OF THE SALIENT POINTS. IT IS IMPORTANT TO REPORT ALL INCIDENTS TO THE POLICE TO ALLOW THEM TO GET A TRUE REPRESENTATION OF WHAT IS HAPPENING ON THEIR PATCH

BY COMPLETING THIS FORM REMINDS YOU OF MOST OF THE IMPORTANT POINTS AND WILL HELP WHEN YOU REPORT THE CRIME BY PHONING 101 FOR NON URGENT CASES OR AT YOUR LOCAL POLICE STATION
